Week 7: Patient Safety and High Reliability Analysis

This Week's Assignment: 

Using the performance improvement tools that you have previously learned in this course, complete an analysis of the organizational assessment data collected with your group members.

• What gaps have you identified in the organizational culture of safety or person-centeredness?
• What opportunities are there to improve their framework for performance improvement? 
• What do you think the root causes are?  (For the purposes of this case study, you can use your own professional experiences to examine this with your group members and make this up. Have fun with it!) 
• Which tool(s) did you use to determine the root causes? 

Complete the Assessment section of the Patient Safety Strategic Plan by summarizing key assessment detail that is important for the leadership team to know, as well as a summary of your group analysis. It is important to be concise while at the same time capturing critical information. Include the tools you use to complete the root cause analysis as an attachment, such as fishbone diagrams or the 5 Why's. (Refer to Chapters 5 & 6 in your text if you need a refresher.)
