Objective: Students will work in small groups to analyze and determine improvements in the culture of safety and reliability in a community healthcare system based upon a given case study. At the end of this 4-week assignment, each group will submit a Patient Safety Strategic Plan using the provided template.
The template is in SBAR format, but the Situation and Background sections have been pre-populated. As a group, you'll complete the Assessment and Recommendations sections. This is an organizational strategic plan, not a department-level plan, and should be high level and succinct.
Key Components to Include:
- An analysis of the gaps in safety and reliability that your group identified from the case study assessment data. (Be sure to include your root cause analysis as an attachment -- Appendix. You can use whichever tool your team feels is most appropriate.)
- A list of short-term recommendations (i.e., those that must be addressed first) and long-term recommendations (to be carried out over the next 3-5 years). Your role as a consultant is to provide the senior leadership team with evidence-based suggestions for improvement and the rationale for each. These recommendations should be high level; the intent is for this strategic plan to provide a foundation for specific performance improvement activities over the next several years.
- Any supporting attachments you think will help to reinforce the message for the senior leadership team and provide future direction. In addition to your references, this could include info graphics, charts, graphs, etc. that will help to provide visual reinforcement for your recommendations. (Assume that most executives will not have time to review the actual references; how can you summarize the evidence in the most visually appealing way?)
Be sure to use the rubric below to guide your work. You can have as many attachments as you think helpful, but limit the strategic plan to no more than 2 pages. One of the biggest challenges in leadership is being short and to the point, while also providing clear direction.

Incorporation of Peer Feedback

Format
Well written, logically developed, and conforms to standard rules of grammar. The plan is concise and succinct, and does not exceed two pages. Supported by a minimum of 3-5 references. Attachments provide visual reinforcement of evidence-based recommendations. Documented using APA format (including Title Page).
