UTA Performance Evaluation Review with Faculty Supervisor
Purpose
Receiving formal feedback from your faculty supervisor is important for your personal and professional growth. It can help you identify areas for improvement, as well as areas where you excel. Meeting in person or remotely provides an opportunity for a richer exchange of information regarding their impression of your performance, ways to capitalize on your strengths, and how to further develop your skills and abilities. It also provides an opportunity to ask questions and get clarification if needed.
Directions
- Meet with your faculty supervisor at the previously scheduled time and location (i.e., campus or via Zoom) to review the supervisor's assessment of your performance.
- Bring a copy of your self assessment to the meeting and be prepared to answer questions they may ask you about your self assessment.
- Ask for clarification and/or examples if you need additional information to help you assimilate their feedback.
- Make sure that you and your faculty supervisor sign the form. Your faculty supervisor will submit their evaluation to your HSC4813 instructor. If you would like a paper copy of the evaluation, ask your faculty supervisor to print one for you.
- After the session, reflect on the differences and similarities between your faculty supervisor's assessment and your self assessment and consider the takeaway messages you can learn from this reflection. You will be asked to share your reflections on this next week as part of the final exam for this course.
Grading
- The performance evaluation submitted by your faculty supervisor counts as 50% of your final grade for this course.
- A score of 48 points is equivalent to 100%, even though it is possible to earn more points than this. This essentially breaks down to earning a rating of "3 - meets expectations" for each criterion listed on the form.
- The maximum score that will be recorded for this assignment is 48 x 2 = 96 (100%). The value in scoring higher than this should be in knowing that you exceeded your faculty supervisor's expectations.
