Schedule Formal Evaluation of UTA Performance
Purpose
Receiving formal feedback from your faculty supervisor is important for your personal and professional growth. It can help you identify areas for improvement, as well as areas where you excel. Faculty (and students) have full schedules, so it is wise to arrange a meeting time in advance of the due date, plus your faculty supervisor will need time to prepare a thoughtful evaluation of your performance.
Directions
- Contact your faculty supervisor to request a time between November 29 and December 3, 2021, to conduct a formal evaluation of your performance as a UTA. The evaluation may be conducted via Zoom or in person.
- Provide your faculty supervisor with a copy of the UTA Performance Evaluation Download UTA Performance Evaluation form immediately after setting up the appointment.
- Meet with your faculty supervisor at the scheduled time to review their evaluation of your performance. Your supervisor will submit your performance evaluation to your HSC4813 course instructor.
Grading
The performance evaluation submitted by your faculty supervisor counts as 50% of your final grade for this course. A score of "3" for each criterion is equivalent to 100% (48 total points). Scores will be doubled when calculating the final grade for the course.
