Volunteer Service Form
Completion of the Volunteer Service Form is the first step to getting set up for your role as an undergraduate teaching assistant (UTA). (Note: The Volunteer Service Form is accessed from the link given above. Scroll down the page to #1 and look for where it says "this link".) Completing this form is necessary to gain access to the FERPA training/quiz and for adding you to the Webcourses for your UTA assignment. Follow the directions listed below or the directions provided to you in your UTA Acceptance Letter Download UTA Acceptance Letter.
- Access the UCF University Compliance, Ethics and Risk Volunteer Services website. Read the page and complete the form. The information you need to complete the form is as follows:
- Department of assignment: Department of Health Sciences
- UCF location: Main campus
- Room number: HS 2, 210
- First name of primary supervisor: (list the first name of the faculty member for whom you will serve as a UTA)
- Last name of primary supervisor: (list the last name of the faculty member for whom you will serve as a UTA)
- Supervisor email: (list the email for your faculty supervisor)
- My volunteer assignment is a: recurring event
- Supervisor phone number: (list the phone number of your faculty supervisor)
- Volunteer assignment start date: August 11, 2021
- Description of volunteer assignment and duties: Serve as an undergraduate teaching assistant, which may include assisting with facilitating class activities and discussions; moderating discussion boards/chats; holding office hours; holding review sessions; assisting with proctoring exams; giving a presentation; reviewing course materials; and other duties related to serving as an undergraduate teaching assistant.

- Will the volunteer access any lab, participate in any research activity, or operate any research equipment: No
- Immediately after completing the form, email Ms. Michele Locke (michele.locke@ucf.edu) to inform her that you submitted the form. Ms. Locke will create a sponsored account for you as soon as the form you submitted has been processed.
- You will be notified when the sponsored account request is received, and you will need to follow the instructions provided in that email notification. This needs to be completed as soon as possible to avoid having the request cancelled.
- Once the account has been created you will receive another email stating that your UCF sponsored account has been assigned and granted access for use. This will allow you to self-enroll in FERPA training. See the FERPA training/quiz assignment for details on how to enroll for this training.
