Module 4 Options for Discussion Question #2

The continuum of care is the complete range of healthcare programs and services from initiation to conclusion available to clients with complex or long-term healthcare needs. Not every type of service or program is available in every community. Some organizations provide multiple services and thus they are referred to as a health care system. Think about the various services available to meet the complex and/or long term health care needs of the client (problems such as homelessness, mental illness, maternal-newborn- child health issues, stroke, cancer, HIV, etc).

Local Healthcare System

Research and discuss a healthcare system in your community. Describe a population served, all of the services available (including nursing) and the settings/environments in which they are offered. Identify any barriers, duplications or gaps that exist.
