Module 4 Options for Discussion Question #2

The continuum of care is the complete range of healthcare programs and services from initiation to conclusion available to clients with complex or long-term healthcare needs. Not every type of service or program is available in every community. Some organizations provide multiple services and thus they are referred to as a health care system. Think about the various services available to meet the complex and/or long term health care needs of the client (problems such as homelessness, mental illness, maternal-newborn- child health issues, stroke, cancer, HIV, etc). In your response to one of the following, please do not duplicate material discussed in your classmates posts.

Local Healthcare System

Research and discuss a healthcare system in your community. Describe a population served, all of the services available (including nursing) and the settings/environments in which they are offered. Identify any barriers, duplications or gaps that exist.
