Objective: Students will use the Model of Improvement to systematically analyze a problem and then develop, implement and evaluate a plan to improve the problem. At the end of the 6 weeks, you will submit a poster to illustrate your improvement process. The poster should include the following elements:
1. Problem and Goal statements. Goals are SMART (Specific, Measurable, Attainable, Relevant, Time-based)
2. Either a Cause & Effect Diagram or Key Driver diagram.
3. A PDSA cycle plan.
4. Baseline data analysis of problem.
5. 2 Run Charts: 1 Process and 1 Outcome measure.
6. 2-3 Lessons Learned.

Rubric
Keep in mind that 27 students have already been assessed using this rubric. Changing it will affect their evaluations.
